Deciding to have a family is incredibly exciting, but the journey can sometimes feel overwhelming, especially when unexpected challenges arise around getting pregnant. As a fertility specialist, I see firsthand the huge range of concerns and questions that so many couples have. Common concerns and what can you do?

1. Blocked Fallopian Tubes

Blocked fallopian tubes can prevent the egg and sperm from meeting, hindering your chances of pregnancy. Be mindful of irregular periods, pelvic pain, or unusual discharge, which can sometimes signal blockages. Consider a diagnostic test like HSG (hysterosalpingography) or laparoscopy if you suspect an issue. In some cases, a procedure called salpingectomy can remove a damaged tube, potentially improving your chances of conception.

 

2. Sperm quality

Sperm quality plays a crucial role in fertilisation. Maintaining a healthy weight, reducing stress, and limiting smoking and alcohol can all contribute to better sperm health. A semen analysis assesses sperm count, motility, and morphology, providing valuable insights. In specific cases, medications like gonadotropins may be used to boost sperm production.

 

3. Age-Related Challenges (Early 40s)

As you enter your early 40s, your fertility naturally declines. However, there are ways to maximise your chances. Supplements can support the corpus luteum (vital for early pregnancy) after ovulation. Additionally, prioritising ovulation monitoring and natural cycles helps capitalise on your most fertile windows. Each individual is unique, and it is crucial to tailor treatment based on your specific health and needs.

 

4. Chromosomal Concerns

Sometimes, chromosomal abnormalities in embryos can lead to miscarriage. Preimplantation Genetic Testing for Aneuploidies (PGT-A) is an advanced tool that analyses embryos before implantation, selecting those with the correct number of chromosomes. While achieving success might require patience and multiple cycles, this technology offers hope for a healthy pregnancy.

 

5. Endometriosis and Fertility

Endometriosis, a condition where tissue similar to the uterine lining grows outside the uterus, can impact fertility. Medications like GnRH agonists or aromatase inhibitors can manage symptoms, while controlled ovarian stimulation protocols during IVF cycles aim to optimise egg retrieval while minimising risks like OHSS. Before considering surgery, carefully discuss potential impacts on your ovarian reserve and long-term fertility with your doctor.

 

6. Prenatal Vitamins

Prenatal vitamins like folic acid are crucial for women before conception, and maintaining a balanced diet rich in essential nutrients provides a strong foundation for a healthy pregnancy.
